I got them a couple months ago and the first thing I noticed was that the right shoe hurt my foot. The stiff mount for the micro adjuster was exactly on a nerve. By loosening the ratchet and making sure the tongue is away from that area it is ok. I hope that will work better with time.
Another thing I don't like is the heel pad. Wearing Speedplay cleats makes the shoe tip so that very little of the heel pad touches the ground at stops. I may have to modify them with my own pad soon.
On the plus side they are light, and the micro adjustment allows for a snug fit. My first Shimanos lasted about five years so I expect these to do the same.

I wear a US 9.5 EEEE shoe, and I purchased the size 44 wide shoes. While not quite as wide as my feet, they're much more comfortable then other cycle shoes I've worn. Although these aren't "Custom" fit shoes, the fine adjustments in the velcro and ratchet on the shoe's upper mean I can make these as tight or loose as I require.
I use these with SPD style cleats and I've been able to use them both on my road bike as well as on a spin bike without trouble.

I normally wear a EE shoe with any athletic shoes I have, and have even worn a 4E (I've always joked that I have "Duck feet" since they are wide and fairly flat). When I first got my bike I had a pair of Adidas that were 44Cm, which is a tiny bit small overall, but they came with the bike (I bought it used from a guy that was about my same size). These would cause my feet to go numb even before I got on the bike! Riding any rides over a couple hours would pretty much ensure that I had no feeling in my entire feet (the numbness starts at the toe box and moves up).
With the Shimano shoes in a Wide size I've not had any of these issues. The fit is very comfortable (though it was weird to have a little hole in the bottom of the shoe where my middle toe is) and I think they look pretty good. I'll be taking them on a 50 mile ride soon and can update the review afterwards if I remember.
